Institutional policy requires that all personnel entering the animal facility be authorized prior to entry.
For a brief or one-time visit: Employees may complete an Observer Waiver to bring in a visitor. The visitor will observe only and will have no contact with animals or hazards. They must not be left unattended in the animal area at any time. The maximum period approved is typically 3 days.
For summer visitors: If an individual must enter the facility for several weeks/months, they will need to be added to an approved animal use protocol.
Unauthorized entry jeopardizes employee health, animal safety, and the security of the institution. If individuals are escorted into the facility without prior approval, your facility access privileges may be restricted.
Do not type additional rooms in the comments section. The system digitally reads these forms and will not catch rooms listed in this area.
To show hazard rooms you must complete all of the below requirements first:
Click the ‘Access’ tab, and select “Requests” from the dropdown menu. Click the number of the pending access request
You will receive an email each time your request is updated.

Please note that many of these steps requires a human response, so please be patient with us.
Since the system is new, some of the historic records may not have been completely transferred. If you have records of the missing items please send training information to EQA, and medical records to your provider.
Some items must be completed on a reoccurring basis, so you may be overdue:
Training NHP training (annually) Rodent Surgery Training (every 3 years)
Medical requirements
All animal users
Medical questionnaire (annually)
Tetanus vaccine (every 10 years)
NHP users TB assessment (annually)
Do you have access to that area of the animal facility?  To check, Click the ‘Access’ tab, and select “My Access” from the dropdown menu. If the room is not listed you need to submit a request
If you cannot get into the building Follow the steps above to request institutional access

At the top right side of the screen click on the link with your name
If you do not meet the basic requirements for access, the system will revoke your access to the related areas. Below are the reasons you may have been revoked.  Current Employee    Your employment is terminated    You did not renew your non-employee ID
 Listed as an Animal User on a protocol
   You are not listed as an Animal User
   Your protocol expired
 Hazard Access
    You are not listed as an animal user under the hazard section of the protocol
    You have not signed all of your P&C forms (every 3 years)
 Mandatory Training Requirements not completed
   Facility Orientation is required within 30 days of receiving access
   NHP training is required annually
   Rodent Surgery Training is required within 60 days of receiving access and every 3 years afterwards
 Medical Clearance not updated
   All animal users
     Medical questionnaire (annually)
     Tetanus vaccine (every 10 years)
   NHP users
     TB assessment (annually)
